Am hoping to fit disc brakes to an older Kona Caldera frame, the bike is of unknown age but has what appears to be a mounting boss though the centers are approx 43mm so a standard IS Shimano adapter for the intended Deore M615 caliper does not fit.

The 43mm standard is an older standard adopted by Trek; I think the item you need is a 'Trek Disco Mount' to convert it up to run the 51mm standard, but as this is a discontinued standard, it may be very difficult to source the part. I would suggest contacting your local Trek dealer as a first step, to see if any such adapters are likely to be available.

If the mount on the forks that the calliper will attach to is also IS (51mm eye to eye); then an 'IS to IS' for the corresponding rotor size (180mm) is required.

Any 6 bolt 180mm rotor will work fine with this set up. The only issue you will have is if you try to run a Floating rotor as the rivets can catch but any normal rotor will be fine.

Shimano and Avid brakes use a different mounting technique and for running a new set of XT brakes; new mounts will be required (in relation to rotor size).
