Answer: 
The 203mm Adapter (sku41260) will enable a Shimano post mount caliper to run a 203mm rotor on an I.S. frame (51mm eye to eye on the brake mounts on the frame).

No it won't change the size of your rotor. To change from your rear 180mm rotor to a 203mm rotor you will need to purchase a 203mm rotor and a mount to suit a 203mm rotor, when buying a mount check it the correct fitting, 'for example' IS to Post or Post to Post...

Yes order the mounts for 180mm rotor size. Make sure to check you order the correct fitting. 'For example' your front mount could be Post to Post and rear could be IS to Post but yours may be different so check this before ordering.

You should decide first what type of your brake mounts on your frame and fork. (IS or POST) after that yes you need some 180mm adapter for 180mm rotors. As I said types depend on your fork and frame type.

Post mount frame to IS caliper.

Hi I have a frame with post mount 74 mm frame mounts , on the chainstay, for the rear brake, I want to attach a standard 51mm IS disc brake (160 mm rotor will be used). This is the opposite of how this adapter is described, but it looks like it might work by the picture. Will it work, if not have you got an adapter that will ?

Answer: 
Likely your frames Post Mounts are designed for use with a 160mm Rotor; however to make the adaption between IS and Post mount, the only option is a IS to POST +20 adaptor which would require using a 180mm rotor.
