If you don't spin at high cadence, have numbness or hot spot issues in toes and ball of foot, and use Speedplay pedals then you have to get this. You can move the centerline of the pedal 14mm back toward the heel and take a lot of pressure off of the ball of the foot. You cannot spin 90+ RPM as smoothly and your ability to pull back with the hamstrings is slightly diminished, but your feet will feel a lot better and you can merrily mash away. They will also lighten the load on the calves somewhat as the pressure point is closer to the ankle and your calf does not have to support the length of the foot so much on the downstroke.
